Contact information is mandatory in case we have specific questions. You may also download a PDF version of the form print it out, fill it in, and mail it in with all the required information.
 

We require a sketch, photo, portrait or pictorial reference of the garment you want. We will not provide you a quote based on a written description. The possibility of miscommunication is too great. We want you garment to be wonderful, not just OK. Also, we will not copy the work of another costumer or designer. This violates an ethical code that dates back hundreds of years.
